An Expert Report Reveals Faulty Practices Leading to Explosions in Silos
A recent report prepared by an expert committee has shed light on the details surrounding the explosion that occurred at the TMO silos in Derince on August 7, 2023. The investigation, requested by the prosecutor’s office and supported by Kocaeli Governor Seddar Yavuz, has revealed that both TMO officials and subcontractor company officials were found to be primarily at fault for the incident.
The report outlines that the explosion was caused by a combination of factors, including a dusty environment, flammable materials, and a series of faulty practices. It was noted that deficiencies to prevent dust explosions and fires had been identified in a 2021 safety assessment but were not adequately addressed. It was also revealed that smoking and careless disposal of cigarettes contributed to the hazardous conditions that led to the explosion.
Specific individuals, including the TMO Kocaeli Chief Manager Kenan Destici, as well as the owner of the subcontractor company and field officer, were found to be at fault for failing to provide proper training, risk analysis, and necessary safety measures.
The report has prompted the initiation of a judicial process, and an indictment is set to be prepared regarding the explosion. It is a tragic reminder of the importance of adhering to safety regulations and implementing best practices to prevent such disasters in industrial facilities.
